Important note about health information. If you are or become a patient of OASIS Regenerative Medicine, health information we create or receive in the course of providing care is protected health information under HIPAA. It is governed by our Notice of Privacy Practices, not by this Privacy Notice. See Section 14.

1. WHAT INFORMATION DO WE COLLECT?
Personal information you disclose to us
In Short: We collect personal information that you provide to us.

We collect personal information that you voluntarily provide to us when you express an interest in obtaining information about us or our products and Services, when you participate in activities on the Services, or otherwise when you contact us.

Personal Information Provided by You. The personal information we collect may include:

Sensitive information, including any health information you provide through our website forms, is never sold, shared for targeted advertising, or disclosed to advertising networks, analytics providers, or social media platforms.

Payment Data. We may collect data necessary to process your payment if you choose to make purchases, such as your payment instrument number and the security code associated with your payment instrument. All personal information that you provide to us must be true, complete, and accurate, and you must notify us of any changes to such personal information.
Information automatically collected
In Short: Some information — such as your Internet Protocol (IP) address and/or browser and device characteristics — is collected automatically when you visit our Services.

We automatically collect certain information when you visit, use, or navigate the Services. This information does not reveal your specific identity but may include device and usage information, such as your IP address, browser and device characteristics, operating system, language preferences, referring URLs, device name, country, location, information about how and when you use our Services, and other technical information. This information is primarily needed to maintain the security and operation of our Services, and for our internal analytics and reporting purposes.

The information we collect includes:

Log and Usage Data. Service-related, diagnostic, usage, and performance information our servers automatically collect when you access or use our Services.

Device Data. Information about the computer, phone, tablet, or other device you use to access the Services.

Location Data. Information about your device's location, which can be either precise or imprecise, based on your IP address. You can opt out by disabling your Location setting on your device.

We process your personal information for a variety of reasons, including:

  • To measure the effectiveness of our advertising. We use advertising and analytics tools to understand how visitors find our website and which marketing efforts lead to inquiries. We have configured these tools so that the specific conditions, treatments, or services you view on our website are not transmitted to advertising networks or social media platforms.

  • To protect our Services, including fraud monitoring and prevention.

  • To evaluate and improve our Services, products, marketing, and your experience.

  • To identify usage trends.

  • To comply with our legal obligations.



3. WHEN AND WITH WHOM DO WE SHARE YOUR PERSONAL INFORMATION?
In Short: We may share information in specific situations described in this section and with the following categories of third parties.

Vendors, Consultants, and Other Third-Party Service Providers. We may share your data with third-party vendors, service providers, contractors, or agents who perform services for us or on our behalf and require access to such information to do that work. We have contracts in place with our third parties designed to safeguard your personal information. Where a vendor processes protected health information on our behalf, we enter into a Business Associate Agreement with them as required by HIPAA.

4. DO WE USE COOKIES AND OTHER TRACKING TECHNOLOGIES?
In Short: We may use cookies and other tracking technologies to collect and store your information.

We may use cookies and similar tracking technologies (like web beacons and pixels) to gather information when you interact with our Services. Some online tracking technologies help us maintain the security of our Services, prevent crashes, fix bugs, save your preferences, and assist with basic site functions.

We also permit third parties and service providers to use online tracking technologies on our Services for analytics and advertising measurement.

How we limit tracking on health-related pages. Pages on our website that describe a specific condition or treatment are configured so that advertising and social media tracking technologies do not report your visit. Advertising conversion tracking operates only on a generic confirmation page after you submit an inquiry form, and that page does not identify which condition or treatment you inquired about.

To the extent these online tracking technologies are deemed to be a "sale" or "sharing" (which includes targeted advertising, as defined under applicable laws) under applicable US state laws, you can opt out as described in Section 10.

Google Analytics
We may share your information with Google Analytics to track and analyze the use of the Services. We have disabled Google Signals and Google Analytics Advertising Features, including Demographics and Interests Reporting, on our property. To opt out of being tracked by Google Analytics across the Services, visit https://tools.google.com/dlpage/gaoptout. 5. HOW LONG DO WE KEEP YOUR INFORMATION?
In Short: We keep your information for as long as necessary to fulfill the purposes outlined in this Privacy Notice unless otherwise required by law.

We will only keep your personal information for as long as it is necessary for the purposes set out in this Privacy Notice, unless a longer retention period is required or permitted by law, including applicable medical record retention requirements.

When we have no ongoing legitimate business need to process your personal information, we will either delete or anonymize such information, or, if this is not possible, we will securely store your personal information and isolate it from any further processing until deletion is possible.


6. HOW DO WE KEEP YOUR INFORMATION SAFE?
In Short: We aim to protect your personal information through a system of organizational and technical security measures.

We have implemented appropriate and reasonable technical and organizational security measures designed to protect the security of any personal information we process. However, despite our safeguards, no electronic transmission over the Internet or information storage technology can be guaranteed to be 100% secure. Transmission of personal information to and from our Services is at your own risk. You should only access the Services within a secure environment.


7. DO WE COLLECT INFORMATION FROM MINORS?
In Short: We do not knowingly collect data from or market to children under 18 years of age.

We do not knowingly collect, solicit data from, or market to children under 18 years of age, nor do we knowingly sell such personal information. By using the Services, you represent that you are at least 18 or that you are the parent or guardian of such a minor and consent to such minor dependent's use of the Services. If we learn that personal information from users less than 18 years of age has been collected, we will deactivate the account and take reasonable measures to promptly delete such data from our records. 8. WHAT ARE YOUR PRIVACY RIGHTS?
In Short: You may review, change, or terminate your account at any time, depending on your country, province, or state of residence.

Withdrawing your consent: If we are relying on your consent to process your personal information, you have the right to withdraw your consent at any time by contacting us using the details in Section 15.

However, this will not affect the lawfulness of the processing before its withdrawal, nor, when applicable law allows, will it affect the processing of your personal information conducted in reliance on lawful processing grounds other than consent.

Opting out of marketing and promotional communications: You can unsubscribe from our marketing and promotional communications at any time by clicking the unsubscribe link in emails we send, replying "STOP" to SMS messages we send, or by contacting us using the details in Section 15. You will then be removed from the marketing lists. However, we may still communicate with you to send service-related messages necessary for the administration of your care, to respond to service requests, or for other non-marketing purposes.

Cookies and similar technologies: Most web browsers are set to accept cookies by default. You can usually set your browser to remove or reject cookies, and you can enable a Global Privacy Control signal in browsers that support it — we honor that signal automatically. See Section 9. 9. CONTROLS FOR DO-NOT-TRACK AND OPT-OUT PREFERENCE SIGNALS
Most web browsers and some mobile operating systems include a Do-Not-Track ("DNT") feature you can activate to signal your privacy preference not to have data about your online browsing activities monitored and collected. No uniform technology standard for recognizing and implementing DNT signals has been finalized, and we do not respond to DNT browser signals.

However, we do recognize and honor the Global Privacy Control (GPC) signal. If your browser or a browser extension sends a GPC signal, we treat it as a request to opt out of the sale or sharing of your personal information, and our analytics and advertising technologies are disabled for your visit automatically. You do not need to contact us separately.

We also honor the "Do Not Sell My Personal Information" control in our website footer. Selecting it disables advertising and analytics technologies for your visits to our site, and no advertising or social media pixel will load. Your choice is remembered on the device and browser you used to make it.


10. DO UNITED STATES RESIDENTS HAVE SPECIFIC PRIVACY RIGHTS?

We do not sell or share sensitive personal information, we do not use or disclose it for targeted advertising, and we do not collect or process it for the purpose of inferring characteristics about you.
How We Use and Share Personal Information
We collect and share your personal information through:

We may disclose your personal information to our service providers pursuant to a written contract between us and each service provider. Where that service provider handles protected health information, that contract includes a Business Associate Agreement.

We may use your personal information for our own business purposes, such as undertaking internal research for technological development and demonstration. This is not considered "selling" of your personal information.

Your Rights
You have rights under certain US state data protection laws. These rights are not absolute, and in certain cases we may decline your request as permitted by law. These rights include:

  • Right to correct inaccuracies in your personal data

  • Right to request the deletion of your personal data

  • Right to obtain a copy of the personal data you previously shared with us

  • Right to non-discrimination for exercising your rights

  • Right to opt out of the processing of your personal data if it is used for targeted advertising, the sale of personal data, or profiling in furtherance of decisions that produce legal or similarly significant effects


Depending upon the state where you live, you may also have additional rights, including rights to access the categories of personal data being processed, to obtain lists of third parties to which we have disclosed or sold personal data, to question how personal data has been profiled, and to limit the use and disclosure of sensitive personal data.

 

How to Exercise Your Rights
To exercise these rights, email us at info@oasislv.org, or write to us at the address in Section 15.

To opt out of the sale or sharing of your personal information, targeted advertising, or profiling, use any of the following:

  • Click the "Do Not Sell My Personal Information" link in the footer of any page on our website. This takes effect immediately and requires no further action from you.

  • Enable Global Privacy Control in your browser. We honor this signal automatically and no further action is needed. See Section 9.

  • Email info@oasislv.org with the subject line "Do Not Sell or Share My Personal Information."


Options 1 and 2 take effect immediately. For requests submitted by email, we will respond within 60 days. If we require more time, we will notify you and may extend by an additional 30 days as permitted by law.

Under certain US state data protection laws, you can designate an authorized agent to make a request on your behalf. We may deny a request from an authorized agent that does not submit proof of valid authorization.

Request Verification. Upon receiving your request, we will need to verify your identity. We will only use personal information provided in your request to verify your identity or authority to make the request.

Appeals. Under certain US state data protection laws, if we decline to take action regarding your request, you may appeal our decision by emailing info@oasislv.org. We will inform you in writing of any action taken or not taken, including a written explanation of the reasons. If your appeal is denied, you may submit a complaint to your state attorney general.

California "Shine The Light" Law. California Civil Code Section 1798.83 permits our users who are California residents to request and obtain from us, once a year and free of charge, information about categories of personal information (if any) we disclosed to third parties for direct marketing purposes. If you are a California resident and would like to make such a request, please submit your request in writing using the contact details in Section 15.


11. NEVADA RESIDENTS
Under the Nevada Privacy of Information Collected on the Internet from Consumers Act (NRS 603A.300 to 603A.360), Nevada residents have the right to direct us not to make any sale of certain covered information we have collected or will collect about them.

Our designated request address for Nevada opt-out requests is info@oasislv.org.

To submit a request, email that address with the subject line "Nevada Do Not Sell Request" and include the email address associated with your interactions with us so that we can verify your identity. We will respond within 60 days of receipt. If circumstances require, we may extend that period by an additional 30 days and will notify you of the extension.

You may also opt out immediately, without submitting a request or identifying yourself, by clicking the "Do Not Sell My Personal Information" link in the footer of any page on our website, or by enabling Global Privacy Control in your browser. Both take effect right away.


12. SMS AND TEXT MESSAGING
If you provide your mobile number and opt in to receive text messages from us, the following applies.

No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Information sharing to subcontractors in support services, such as customer service, is permitted. All other use case categories exclude text messaging originator opt-in data and consent; this information will not be shared with third parties.

You may opt out of SMS messages at any time by replying STOP to any message. Reply HELP for assistance, or contact us at info@oasislv.org or +1 702-842-0052. Message frequency may vary. Message and data rates may apply.

13. DO WE MAKE UPDATES TO THIS NOTICE?
In Short: Yes, we will update this notice as necessary to stay compliant with relevant laws.

We may update this Privacy Notice from time to time. The updated version will be indicated by an updated "Last updated" date at the top of this Privacy Notice. If we make material changes, we may notify you either by prominently posting a notice of such changes or by directly sending you a notification. We encourage you to review this Privacy Notice frequently.


14. HEALTH INFORMATION AND HIPAA

OASIS Regen PC is a covered entity under the Health Insurance Portability and Accountability Act of 1996 ("HIPAA").

Protected health information is governed separately. Health information we create or receive in connection with providing, arranging, or billing for your health care is protected health information ("PHI") under HIPAA. PHI is governed by our Notice of Privacy Practices, not by this Privacy Notice. Where this Privacy Notice and our Notice of Privacy Practices differ with respect to PHI, the Notice of Privacy Practices controls.

You may obtain a copy of our Notice of Privacy Practices by requesting one at info@oasislv.org or at our office.

What this Privacy Notice covers. This Privacy Notice governs information collected through our website and marketing activities, including inquiries from people who are not yet patients.

We do not disclose PHI for advertising. We do not sell PHI, and we do not disclose PHI to advertising networks, social media platforms, or data brokers. We do not use PHI for marketing purposes without your written authorization where HIPAA requires one.

How we limit tracking on health-related pages. Advertising and social media tracking technologies are configured so that they do not report visits to pages describing a specific condition or treatment. Conversion tracking operates only on a generic confirmation page that does not identify the condition or service you inquired about, and the address of the page you came from is not transmitted.

Vendors who handle PHI. Where a vendor processes PHI on our behalf, we enter into a Business Associate Agreement with that vendor as HIPAA requires.

Your HIPAA rights. If you are a patient, you have rights under HIPAA including the right to access your medical records, request corrections, request restrictions on certain uses and disclosures, receive an accounting of disclosures, and file a complaint. These rights are described in our Notice of Privacy Practices. You may also file a complaint with the U.S. Department of Health and Human Services Office for Civil Rights at https://www.hhs.gov/ocr/complaints. We will not retaliate against you for filing a complaint.
